Case:	An 89 year old male presented to the outpatient department with progressive dyspnea and chest pain.
History:	The patient had a history of a Non-ST-Elevation (NSTE) myocardial infarction (MI) in 2007 and 2008 on the base of a known coronary heart disease (CHD). He underwent a bypass operation in 2008. Chronic anemia is also known.
Investigation:	A standard chest x-ray was performed.
X-Ray Findings:	The image shows a pleural effusion on the right side. There is also a right hilar mass with a diameter of 14 cm. Minor pulmonal venous congestion.
Question:	What is the anatomic situation and pathology?
